Implementation of an electronic patient managed health record system for patients with cancer.


Your participation in the study is entirely voluntary. You may refuse to take part in the study and you may stop participating in the study at any time without penalty or loss of the benefits to which you were entitled to before taking part in the study. You do not have to take part in this study to be treated for your illness or condition.

Eligibility
To participate in the study you must:
1. Be diagnosed with cancer
2. Be at least 18 years old
3. Provide informed consent that you are willing to participate in the study
4. Have access, and be able to use the Internet.

You are encouraged to discuss your participation in the MyQuest study with your doctor.

Health Care Providers

Dear Medical Practitioner/Physician,

The patient who has notified you about this website has decided to participate in a federally funded cancer trial known as MyQuest.com.au. The trial is being carried out by consortium members Virtual Cancer Centre, Silverchain, Hollywood Private Hospital, St John of God Hospital and Sir Charles Gairdner Hospital - Radiation Oncology Department.

This trial is in an online, patient controlled, personal health record and we need your participation as their caring physician to be involved in the trial process. It will require minimal effort on your behalf as we are aware that you are under tight time commitments.

With your registration and confirmation on your medical credentials, you will be added into a list of available providers which patients using MyQuest are able to link to and allow you access to their personal health records. The aim of this project is to make the patients information easily available to you and other providers so you can spend your time treating your patient, better utilizing your time with patient information always available.
